To take benefit of this scheme please as follows:

• Download the FORM.
• Fill it with signature of member lady.
• Post it to address written on form.
• Pay for Rs.300/- (Rupees Three Hundred Only) directly in Bank detail given below either by CASH or through NEFT or draft of Rs. 272/- (Rupees Two Hundred Seventy Two Only) OR MULTI CITY/AT PAR cheque in favour of "THE ORIENTAL INSURANCE COMPANY LTD." shall also accepted.
• Attached the pay in sleep or NEFT acknowledgement (UTRN) along with form as a proof of payment.
• Write your phone number and email ID at the bottom of the second page.
